2 of 4 B-822-FITAP - PO HOW TO VERIFY RELATIONSHIP TO OTHER QUALIFIED RELATIVE
Once the parental relationship has been established, use documentary proof to establish the child's relationship to the qualified relative. In addition to the documents listed in section B-821-FITAP, the following may be used:
● family bible,
● church records,
● U.S. Census records,
● insurance policies over 3 years old, or
● legal document executed by the court, such as a custody order, which states the qualified relative's relationship to the child.
If none of the above documents is available, the relationship of the qualified relative to the child may be established by securing:
● the qualified relative's written statement as to how he is related to the child, and
● a written statement attesting to the relationship from another individual in a position to know the facts of the situation.
Determine how the collateral is knowledgeable of the client's situation and document this *** in Case Notes. If the collateral is not interviewed in the parish office, contact the collateral by telephone to obtain the documentation of how he knows of the relationship of the qualified relative to the child.
II. PROCEDURES
To request verification of Relationship, select ‘Pending Verification’ or ‘Client Statement’ on the Relationship – Details screen for those relationships that have not been verified. To avoid a request for verification for household members in error, select ‘Reliable Reference’ as the relationship verification type when documentation is available for one of the members and not the other member as relationship has not been verified between those household members. Example: Mother applies for herself and two children. The agency has a birth certificate for the first child only. LITE is updated to reflect the birth certificate contained in OnBase for that child. However, verification is still needed for the second child. Data collection in LITE on the Relationship – Details chart would be updated as follows:
