B-821-FITAP - PO HOW TO VERIFY RELATIONSHIP TO PARENT
Relationship of a child to his parent(s) is primarily verified through a birth certificate. If the birth certificate is not available, other documents may be used if they include the name of the child and the parent(s). These documents may include:
● baptismal certificate,
● documents from the United States Citizenship and Immigration Services (USCIS) and/or identifying information provided in correspondence from a Voluntary Agency,
● licensed midwife records, or
● medical records.
Note: A birth card which does not include the full name of the child, full name of the mother and/or father is not sufficient verification of relationship. Other supporting documentation as outlined above and at B-822-FITAP must be obtained.
If the father's name does not appear on the birth certificate, the determination of his relationship to the child may be established by one or more of the following:
● a court judgment which declares the man to be the child's father;
● documentation that the child receives financial benefits based on circumstances of the father, i.e., Social Security, VA, military allotment;
● documentation that the child is claimed by the man as his dependent child for income tax purposes;
● paternity test results that verify the man to be the father; or
● the man is named by the mother and:
o the man verbally acknowledges paternity to the agency, or o the man signs an acknowledgement of paternity, or o at least two references or collaterals state in writing that the man has acknowledged to them that he is the child's father.
