Payments made directly to or for a vendor — The specific amount for a payment provided to or for a vendor requires verification of costs and how the payment can be accepted. Examples of appropriate verification required from a vendor: • Request for move-in costs at rental home: Verify information such as property ownership (can use assessor and tax records) to determine appropriate benefit level or for payee purposes; verify details such as who is listed to be in the home and rental payment terms. A rental agreement or lease is not required as verification, but it is always encouraged that the survivor obtain a written agreement for their living situation. • Request for truck/trailer rental: Verify the cost of the rental, the number of miles to be driven. Request information about how many trips the survivor believes they need to make to cover costs. • Request for initial or past due utility payment: Verify the cost for initial set up or for past due bills obtain the entire past due bill to ensure payment is made only for the intended need. If ODHS does not receive all required verification, or the verification was not accurate, the payment request is denied. Denied payments must be notified by providing the system generated Notice of Action TA-DVS (DVS-110). Only mail the notice if there is a safe address. If there is no safe address, case note.
