857 Chapter 4:Working with families • Section 14: Engagement in REP Services (07/2026) Note: Participants in REP services may need language interpretation services to support conducting home visits, providing screenings, assessments, or evaluations. Please ensure that language interpretation or document translation services are used, as appropriate, to assist the participant(s) through the re- engagement process. The intent of this community visit is to provide services and options for participants during the re-engagement process:
A community visit must be attempted within 30 days prior to a grant reduction. (An attempted community visit may be done after re-engagement staffing but must be done prior to team determination.)
Prior to community visit, ODHS should send a community visit letter to the participant. Note: This does not have to be on an official ODHS form such as the DHS7849 (JOBS Appointment Notice). Offer any screenings, assessments or evaluations that have not been completed or determine whether an updated or further screening/assessment may be needed (e.g., A&D/MH screening, Learning Needs Screening (LNS), DV screening or Physical Health Needs screening): • The LNS only needs to be completed once; however:
If the participant scored 12 or more, we would need to see if follow up was offered. If not, we must offer the follow up before moving forward in the re-engagement process;
If the participant scored less than 12 on a previous screening, there may be other indicators requiring additional learning needs screenings; • Screenings can be offered in person, over the phone or in writing. Regardless how the screening was offered, it will need to be documented in the TRACS Testing Page that an offer has occurred. • A screening is considered completed when it has been:
Offered – Accepted – Conducted – Documented;
Offered – No Show – Documented;
Offered – Refused – Documented. If a screening is refused, the refusal must be documented in TRACS. Note: GAIN-SS and LNS should only be administered by ODHS staff or designees that have met ODHS training requirements for administering these screenings. These tools should be administered one-on-one between a participant and ODHS staff or designee with appropriate follow up provided by ODHS at time of screening. The LNS must be conducted where ODHS staff or designee ask the questions verbally and records the response. The participant may be given a copy of the screening tool. A DV screening is looking at information regarding the participant’s current safety. Review the following items on a case for potential documented or reported instances of DV or other potential safety risks: • ONE Case (be sure to do an advanced search to determine if there are past or current TA-DVS cases associated with the participant);
