164 (07/2026) Chapter 2:Eligibility • Section 3: Individual information PO Box 1108 ACP 0999-19 Salem, OR 97308 Participants of the program have completed safety planning with a local domestic violence service provider or district attorney-based victims’ assistance program. For further information about the program, go to DOJ Address Confidentiality Program. To apply for the program, the survivor will need to contact the local domestic violence and sexual assault service provider or the local crime victims’ assistance program through their local district attorney. The survivor will work with an application assistant who can help them decide if the program is appropriate for them. • In ONE, choose ‘Yes’ to “Participating in Address Confidentiality Program” and input the ACP number.
All ACP participants will pre-populate Salem as their city and Marion as their county.
lf the ACP Number is only 5 digits, best practice is to input a zero before the number. • lt is best practice to view the ACP card or confirm the ACP number. Add a case note indicating that the ACP card was viewed, or number was confirmed with the participant or DOJ, when able.
We can email or call to verify ACP enrollment at acp@doj.oregon.gov or via phone at 503- 373-1323. This is also helpful if the survivor does not know their participant number. • lmportant: Delivery of mail to ACP addresses takes additional time. While the ONE system ensures that enough time is given when notices are generated, if mailing timely notices outside the system, allow an additional 5 business days (see OAR 461-175-0050). Reminder for SNAP Cash-out payments: An exception needs to be requested for anyone living in cash-out county but using an ACP address. Send an email to SNAP.Policy@odhsoha.oregon.gov requesting Cash-out be manually updated for this case. Include in the email the participant’s name, case number and preferred issuance method (direct deposit or case EBT). Reminder for child-care: Payments issued through the Direct Pay Unit system where care is provided by someone using an ACP address, the maximum payment rates will default to Region A. Reminder for medical benefits: lmmediately following an authorized approval or reported change to add ACP, workers must contact Client Enrollment Services (CEC) to request a CCO enrollment exception, known as a service area exception (SAE), for anyone who does not physically reside in Marion County, so they can be enrolled in the appropriate CCO in their residential county. The email should be sent to DMAP CES and include the following information in the email: Subject line: ‘Medical Exception Needed – ACP Program’ In the body of the email include: • Participants’ names • Case number
