1289 (07/2026) Chapter 8: Examples • Chapter 8: Examples Example 5: Ani is sent a Periodic Report on the first of August, due the end of August. No response is received and on September 1, ONE sends a reminder notice of the extended due date of 9/10. He does not respond, and a decision notice is later issued letting him know his SNAP case will close September 30. November 5, Ani sends in his completed and signed Periodic Report. Because it was not received before the end of October (discontinuance month), it is treated as a new request for SNAP and November 5 is the filing date. Ani must complete the application process. Example 6: Pedro sends in his completed Periodic Report with information that his earned income has increased from $1000 to $1400 a month. He does not include income verification. Because the increase is more than $125, verification is required, and ONE will send an RFI. Example 7: Ozzie sends in his completed Periodic Report with information that he is now married and requests SNAP for his new wife and her child. Ozzie reports that she has earned income but no verification is provided. ONE will issue an RFI. Section 2: Acting on reported changes Medical Reporting Requirements Individual program example(s) OSIPM Prospective Changes Example 1: Dixie calls ODHS|OHA on May 12 to report winnings in the amount of $5,000 received on May 5. Dixie tells ODHS|OHA she has already spent all of her winnings. Even though the winnings exceed the $2,000 OSIPM resource limit, since the resource is already gone, there is no action to take on the case. Looking prospectively ahead to June, a reduction would be inappropriate. Example 2: Dixie calls ODHS|OHA on Nov 19 to report a family member will be moving into the home on Dec 5. The family member meets all eligibility requirements for OSIPM. The family member is added to Dixie’s case and OSIPM is approved effective Jan 1.
