1288 (07/2026) Chapter 8: Examples • Chapter 8: Examples There is no requirement for individuals to submit the blank pages if the box for no changes has been checked. However, all pages submitted need to be uploaded into ONE, including any blank pages received. When a Periodic Report is returned in the Reinstate month: • If the PR is complete, reopen and prorate if the EDG is eligible for SNAP. • If the PR is incomplete, treat as a new filing date. Set up the interview and pend for necessary documentation or information, allowing the usual 30 days. If the requested information is received during the reinstate month and the EDG is eligible for SNAP, reopen the closed cert period and prorate the reinstate month’s benefit. If not received during the reinstate month, continue to treat as a new filing date which will assign a new cert period if approved. Note: When prorated, the benefit is prorated from the date the Periodic Report is received and considered to be complete. This means the date the form is complete, signed and all requested verification and/or information is received. Other resources Periodic Report QRG: Includes policy guidance, ONE system steps for processing the periodic report tasks, and support staff steps for completing the form in person or over the phone. SNAP Example 1: Melissa returned her Periodic Report timely, signed, marked the box ‘I do not have any changes to report’ and left the rest of the form blank. All income and deductions on the case remain the same until the next Periodic Report, renewal, or reported change. These include the excess shelter deduction or homeless shelter deduction, medical and childcare deductions, and court ordered child support already recorded in ONE. Example 2: Jordan returned his Periodic Report timely, but reported his earned income increased by $550 a month. His earned income in ONE was currently below 130% FPL but the earned income reported on his Periodic Report moves him above 130% FPL. ONE will send an RFI requesting verification of his new earned income. Example 3: Betty is sent a Periodic Report on the first of August, due the end of August. August is the 5th month in her 12-month cert period. No response is received, and on September 1st ONE sends a reminder notice of the extended due date of 9/10. She does not respond, and a decision notice is later issued, letting her know her SNAP case will close September 30. Betty then sends in her completed and signed Periodic Report on September 16, indicating ‘I do not have any changes to report’. Her SNAP will continue through the end of the 12th month of her certification period. Example 4: Leiloni is sent a Periodic Report on the first of August, due the end of August. No response is received, and on September 1, ONE sends a reminder notice of the extended due date of 9/10. She does not respond, and a decision notice is later issued letting her know her SNAP case will close September 30. October 5, Leiloni sends in her completed and signed Periodic Report. SNAP is to be restored to continue the certification period and October benefits will be prorated from 10/5.
