1287 (07/2026) Chapter 8: Examples • Chapter 8: Examples 6-month intervals after earned income is added to ONE. If not processed by the end of the 6th, 12th, or 18th month, SNAP will discontinue. • A signature is required to process this report. • This means even if the Oregonian is not reporting any changes, they would still need to check that box indicating “no changes” and still sign the form. The Periodic Report will be requesting information about the following changes from the SNAP household: • All changes in HH composition, such as the addition or loss of a HH member • If the HH has moved the form will ask for updated information on the new shelter costs and utility costs if any. • It will also request information on the new address and any new contact information • Changes in the source of income, so a different source than what was entered into ONE • Changes to the amount of income • If someone joins the filing group and has income, this is a new source, and we will need verification of their income • Verification is required when the total of all earned income changes by more than $125 per month from the amount currently used in the SNAP budget. • Verification is required when unearned income changes by more than $125 per month from the amount currently used in the SNAP budget. When a Periodic Report is received but not signed, it is incomplete. An RFI must be sent to request the completed, signed PR. The Periodic Report form (SNP-069) is considered complete when the form is signed on page 2 by the head of household or an adult member of the SNAP filing group, all requested verification has been submitted, and: • One of the two checkboxes are checked, indicating if there are or are not any changes, • When neither of the two checkboxes are checked, but the resto of the pages are submitted, indicating which items are changed or not changed, or • When the checkbox has been checked indicating there are changes reported, and the rest of the pages have been completed and submitted. When neither of the two checkboxes have been checked, and the rest of the pages are not submitted or are partially or completely blank, the report is not considered complete. Note: When SNAP has been discontinued because the Periodic Report was not processed, the action taken will be different depending on the date the report is considered to be a complete report submitted. If the Periodic Report is turned in but is not complete (missing signature, verification or information), that is not the date we consider it submitted. When the last of the required items are turned in, we then consider the Periodic Report to be submitted. Example: A Periodic Report is turned in on 3/15 and an RFI is issued for income verification. SNAP discontinues on 4/1 and the verification is turned in on 4/5. When reopening SNAP, the Periodic Report was submitted late (after discontinuing) and the date submitted is 4/5, not 3/15.
