1109 Chapter 7:Worker guides • Section 4: Child Care worker guides (07/2026) This worker guide lists the primary forms and pamphlets used in the Employment Related Day Care (ERDC) program. For more information about any of the below mentioned forms, please e-mail ChildCare.Policy@DELC. Oregon.gov. Form number Title and information ODHS 0862 Change Report for Employment Related Day Care (ERDC). This form is used by individuals to report changes when they are receiving ERDC benefits. ODHS 7476 Employment Related Day Care (ERDC) and Supplemental Nutrition Assistance (SNAP) Application. This form is a way for ongoing ERDC and SNAP recipients to renew ERDC and SNAP benefits. ODHS 7486 Special Need Child Care Request. The form may be given to the family electronically, by mail or in-person. It is a resource for families receiving ERDC who have a child with special needs and are requesting the Special Needs Rate (SNR) payment type. The use of this form is optional, it allows families to explain why their child qualifies for the SNR and gives examples of allowable verification types. The form may be submitted to ODHS, though it is not mandatory. Medical documentation is needed to confirm eligibility for the Special Needs Rate. 7492 Child Care Provider Guide. The Child Care Provider Guide helps educate and inform child care providers and families. This guide is given to providers and families when the caretaker applies for ERDC benefits, and can also be given to the provider as a resource at any time it is requested. It is automatically mailed to newly approved providers and to all active providers when a significant change is made to the guide. The Child Care Provider Guide gives information about provider requirements, the listing process, billing, ERDC payment rates, instructions, and samples of completed listing and billing forms. There is additional important information included, such as pre-service trainings, recognizing & reporting child abuse, and preventing the spread of infectious disease. 7494 Child Care Provider Listing Form. The Child Care Provider Listing Form is the first step in a provider becoming listed and approved to receive ERDC payments. The listing form is completed by the child care provider and is submitted to the Direct Pay Unit (DPU) for processing. Based on the information provided on the form, DPU determines whether or not the provider meets DELC requirements. If there is already an open ERDC case, and if the provider is not yet approved for ERDC payments, the worker should fill out the top portion of the form to establish a child care connection once the provider has been approved. If the Branch Use Only section of the form is not filled out with the customer information, it is treated as a ‘prelisting form’ and can still be used by the provider to get into an approved status to receive ERDC payments. Please refer caretakers and providers with any questions regarding the listing process or the Child Care Provider Listing Form to contact the Direct Pay Unit (DPU) at 1-800-699- 9074.
