1110 Chapter 7:Worker guides • Section 4: Child Care worker guides (07/2026) Form number Title and information 7494E DELC Child Care Provider Letter. This form can be given to the caretaker which serves as confirmation of the family’s ERDC eligibility, and that ODHS has been notified of the caretaker’s choice of provider and their intended use of the provider. When a caretaker informs ODHS of their chosen provider, a worker must verify in DHR whether the provider is an approved and listed ERDC provider. If the provider has an “A” list status in DHR, a worker will complete an electronic provider connection template email and send it to DPU. The connection process and mailing of the child care billing (CCB) form can take time, so the worker should ask the caretaker if they would like this form to give to their provider showing their ERDC eligibility. If requested, a worker will fully complete this form and provide it to the caretaker. The form is meant to reassure the provider that they can begin offering care in confidence, knowing payment will be made for the care they provide. The form includes information such as the caretaker’s approved monthly hours, copay, start date of care (if previously discussed with the provider) and renewal date. 7496 Provider Report Form. This form is used by child care providers to report changes in name, address, phone number, household composition; or to report arrests, convictions, or involvement with DHS Child Welfare. A perforated copy of this form is also available in the Child Care Provider Guide (DHS 7492). The Direct Pay Unit (DPU) may also mail these forms to child care providers upon request. Child care roles Worker responsibility • Determine eligibility (ERDC) • Determine if there is an unpaid copay:
Review the case notes, Notice Reasons or the Disqualification Information screen (on the terminated authorization) to determine if there is an unpaid copay;
From a blank DHR screen type “DPCS,Case#” and press enter. Check for a “N” in the Copay Met field on DPCS;
If the Copay Met field shows “N”, the family’s request to continue with ERDC at renewal (or when reapplying at a later date) will be denied until the copay has been paid, the provider agrees satisfactory arrangements have been made, or after three years have lapsed from the month the copay was not met. • Discuss child care need:
What is their work or school schedule – is it consistent or variable? Do they work daytime, evenings, weekends, overnight? Enter the schedule in ONE and case note anything that needs additional explanation.
Are there two parents in the home – what days/times do their work or school schedules overlap? Make a case note of the schedules and enter them on the caretaker schedule screen to review for overlapping hours.
What are the ages of the children? This will help determine the hours of care and child care cost. For
