This includes verification from the following sources (not all inclusive): • ESD/ANSRS • Social Security Acquisition Regulation “SSAR” • BENDEX • SDX • Nevada Vital Statistics • NV KIDS • WebIZ (immunizations) • AMPS Note: Previous quarterly wages may not be used to determine income for SNAP or TANF eligibility as it does not provide a 30-day wage history. Federal Data Services Hub (FHUB) FHUB income verification requests may only be initiated during medical certification or redetermination to determine medical eligibility. Once received, the information can also be used to establish SNAP and/or TANF eligibility when combined with an initial medical request. An FHUB income verification request is not required at medical certification or redetermination if the income source used to determine SNAP/TANF eligibility is less than six months old and the income reported on the medical request reflects less than a 20% change from the income currently being budgeted on the open SNAP/TANF case. FHUB income verification requests cannot be initiated at any other time to resolve income discrepancies or questionable information for SNAP/TANF even when there is an open medical component. 210.1.2 Verification Obtained Directly from the Household (TOC) If the required verification is not available through an electronic source or a collateral contact, the household is responsible for providing verification or evidence needed to prove their circumstances. This includes, but is not limited to, paycheck stubs, rent receipts, utility bills, bank statements, birth certificates, Social Security cards, alien registration cards and driver’s licenses. This also incudes Division forms (e.g. 2074, 2488, etc.) used to verify any eligibility factor.
