This very important phase of the participant’s transition from welfare to work is the point of determination as to whether or not the participant will become self-sufficient, and every effort should be made to work with the participant through this difficult period. The case manager and participant may be able to resolve some problems without a disruption in the participant’s work activity. Any discussion between the case manager, the participant and/or the employer/component provider must be documented on the Maintain Participant Free-Form Remarks PACR screen. The MDHS-EA-376, Client Contract/Action Report form will only be used if JAWS is not available. Each MDHS-EA-387, TWP MDES/MDES Communication Form received from the MDES interviewer must be addressed and documented the same or next day by the case manager. Job Readiness Trainer (JRT)/Employment Coordinator (EC) The JRT/EC will monitor the participant in job readiness, job search and job placement.
