5. Match Reason: New Application, Recertification, Add Household Member, or Other;

6. Benefit Closure Date - The date that benefits ended. (If state is initiating state, it is the date the benefits are denied or ended. If state is matching state, it is the final disposition date);

7. Recent Certification Months - The last three recertification months;

8. If individual is vulnerable;

9. Invalid Match Reason - Incorrect individual information, system error, potential identity theft, or other. Comments are only expected if invalid match reason is other;

10.Initial Action Taken and Date - When the Send RFI Notice is triggered during the nightly batch run then Notice Sent will be populated; and

11.Final Disposition and Date.

E. NAC Match Information Received, displays match details for an individual record from the matching state and will include:

1. Matching State;

2. Contact Email;

3. Contact Phone Number;

4. Case Number (for out of state case);

5. Benefit Closure Date - The date that benefits ended. If state is initiating state, it is the date the benefits are denied or ended. If state is matching state, it is the final disposition date;

6. Recent Certification Months - The last 3 recertification months;

7. If individual is vulnerable;

8. Invalid Match Reason and Comments;

9. Initial Action Taken and Date; and

10.Final Disposition and Date.
