If the State uses a MIS system, describe the E&T related data that is tracked and stored in those systems (e.g. referrals, noncompliance with program requirements, provider determination, etc.), and whether the system(s) interact with each other. CBMS is the State’s system of record for CDHS and all EF POs. CBMS tracks and stores all E&T-related data, including: 1. Referrals and provider determinations (PDs) 2. Exemptions and noncompliance with program requirements 3. Self-reported demographic information 4. Client participation hours and component assignments 5. Case notes and assessments 6. Individualized Service Plans (ISPs) 7. Employment and training outcomes 8. Supportive services and payments 9. Referrals and other relevant case details All EF POs enter data directly into CBMS, which ensures consistency and supports coordination with other CDHS-administered programs. CBMS interacts with eligibility modules to streamline case management and allow for integrated tracking of client progress and program compliance. Describe the State agency’s process for monitoring E&T providers’ program and fiscal operations. Include plans for direct monitoring such as visits, as well as indirect monitoring such as reviewing program data, financial invoices, etc. The State E&T Team uses an oversight plan to monitor the EF POs program and fiscal operations. The oversight plan process is critical in identifying general best practices, program performance, and areas of improvement through quarterly desk-level reviews and follow up meetings as necessary. These meetings and reviews are for one or more of the following program areas: 1. EF internal referral and reverse recruitment and referral processes 2. EF program design and case management 3. EF performance, data, and trends 4. EF quality assurance processes 5. EF case file reviews and documentation 6. EF PO plan and policy reviews 7. EF allocation, expenditure levels, and fiscal trends 8. In person or virtual observation of an EF orientation or initial appointment, workfare assignments, and general program facilitation 9. Applicable contract requirements 10. Supportive payment issuances 11. A thorough examination of expenditures and spending of an EF PO The oversight plan requires a quarterly case file review, which includes the case management and program components listed above for all EF POs. An EF PO who rates below expectations is required to participate in a management evaluation to evaluate how the EF POs are assisting SNAP clients in gaining skills, training, work, or experience that will increase their ability to obtain long-term, sustainable employment. The State E&T team reviews each assessment with the EF PO monthly, and all assessments and actions taken because of the assessment are archived. The State E&T will conduct at least 12 management evaluations this year, utilizing recommendations from internal audits. In addition to the oversight process listed above, the State E&T team reviews a series of program engagement and case management reports with EF POs. Although there is a prescribed oversight process with monthly and quarterly checkpoints, areas of concern are addressed with EF POs as soon as they arise. 19
