7.05

Coordination With Community Resources

POLICY:

The Local Agency’s outreach activities should be coordinated with other health, education and social service programs in the community.

PROCEDURE:

A. Referral to Medicaid

The Local Agency must provide written information on Medicaid on at least one (1) occasion to each adult participant in and/or applicant for the WIC Program. The written information will provide both the address and phone number of the local Department of Health and Human Resources (DHHR) office. (See Policy 7.05, Attachment #1)

B. Referral to Other Health, Education and Social Service Programs

The Local Agency must refer WIC applicants and participants to other health, education and social service programs in the community, including those programs with whom the West Virginia WIC Program has written agreements which require referral (see Policy 10.02, Coordination of Services).
