20.5.9.D Replacement DoHS will replace up to the total allotment of the Summer EBT issuance for instances of food loss (purchased with SEBT) due to a disaster or household misfortune. Mass power outages are not required to be verified. For individual power outages, the outage must have lasted for at least 4 hours to qualify for replacement. When the Governor of West Virginia declares a State of Emergency (specific counties or statewide), the requirement that the household be without power for a minimum of four (4) hours is considered automatically met for the purposes of Summer EBT benefit replacement. No additional verification of the four-hour requirement is needed during the period covered by the declared State of Emergency. The student’s guardian may request replacement more than once but may never receive more than the total of the summer allotment. The student’s guardian must complete a DFA-SNAP-36 form and submit it to the local county DoHS office. The form must be completed within 10 days of the reported disaster or household misfortune. The Family Assistance Policy unit will review any questionable replacement requests and make the final determination for SEBT replacement. Once approved for SEBT replacement, the student family will receive the replaced benefit within 30 days of the request being processed. 20.5.9.E Expungement Expungement for Summer EBT cases will be 122 days following issuance of the SEBT benefit. The issuance date is the date benefits are loaded onto the EBT card. Student families will receive a notice 30 days prior to expungement. Student families are encouraged to use all SEBT benefits prior to the 122-day expungement.
Note: The DFA-SNAP-36 form is a DoHS form only. WVDE will not collect requests for SEBT replacement. The student family must contact the local DoHS office for a replacement request.
