The department shall request a waiver, or an amendment to the waiver requested under par. (a), from the secretary of the federal department of health and human services to require all of the following: 1.

As a condition of receiving services under par. (a), parental notification for family planning services for any female under 18 years of age.
(2)#
The department to determine eligibility to receive family planning services under par. (a) for a female under 18 years of age using the family income of the female’s parent or guardian instead of only the female’s income.
