ICAN’s case management services support E&T participants by providing individualized, participant-centered support based on assessed needs. Case managers work with participants to identify both initial and ongoing needs, offering the level of support that best fits each individual. Services are flexible—participants may receive as much or as little support as needed throughout the program. To ensure timely and consistent access to resources and activities, participants are expected to engage with their case manager and employment team at least once per month. These check-ins can range from formal team meetings to brief email updates, depending on the participant’s preferences and situation. for ICAN-RU staff utilizes the Goal, Plan, Do, Review and Revise (GPDRR) coaching framework to help participants identify goals that are related to employment, training, and education. Case managers will support participants to overcome and address barriers that prevent goal achievement. Case managers can provide financial support to participants such as paying for car insurance/repairs, books or paying for a course to help support goal achievement. How does the SNAP State agency ensure the case management services offered do not act as an impediment to successful participation in E&T? ESD ensures case management services do not impede successful participation by maintaining flexibility in both the mode and frequency of service delivery. Case management is tailored to meet each participant’s needs, ensuring they have access to the necessary support and resources to succeed. Case management services help the participant identify goals that are meaningful to them and their families and that support empowerment and self-motivation since the goals are being created by the participant. Case managers also follow the coaching framework alongside the participant while they make steps toward goal achievement. They help strategize steps to overcome barriers and obstacles that prevent goal achievement and can provide additional support as requested. 1/29/26, 10:27 AM State Plan VT FY26 - Original Submission https://dam-fns.usda.gov/sites/default/files/microsite/snapet-vtfy26stateplan//index.html 42/159
