2100.5.8 Facility Requirements When the Resident Leaves

When the resident leaves, the facility will: End acting as the authorized representative (if applicable); Return the EBT card to the resident when an employee of the facility is the authorized representative; Advise the resident of their reporting requirement by providing an ESD 200, Change Report ; If spent, return half of the resident’s 3SVT benefit according to DCF procedures when the resident leaves the facility before the 16th of the month; Return the EBT card to the State if unable to return the EBT card to the resident.

Facilities acting as authorized representative for the household are responsible for all: losses or misuse of benefits, and overpayments occurring while acting as authorized representative.
