2100.1.2 Eligible Addiction Treatment Centers

Eligible addiction treatment centers must be: certified by the USDA FNS as a retailer, or a private nonprofit organization or institution, or a publicly operated community mental health center funded or eligible for funding under Part B of Title XIX of the Public Health Service Act.

The following addiction treatment centers are eligible: Grace House Magee House Mandala House Maple Leaf Phoenix House Rise Serenity House Valley Vista Teen Challenge Note : Teen Challenge participants may move between Vermont, New Hampshire and Massachusetts. Verify the participant is not receiving benefits in New Hampshire or Massachusetts before issuing 3SVT. Brattleboro Retreat Note : Brattleboro Retreat offers several treatment programs. If a customer reports living at Brattleboro Retreat, we must confirm they are in an addiction treatment program. If at Brattleboro Retreat for another program, then the customer is not eligible for 3SVT.

If the treatment center’s authorization isn’t known, email AOPS . AOPS will determine if the treatment center is eligible.
