100.4.3 Discrimination Complaint Requirements

Discrimination complaints must be filed not later than 180 days from the date of the alleged discrimination. The complaint must contain the following information: The name, address, and phone number of the person alleging discrimination, or other means of contacting this person. The location and name of the office which is accused of discriminatory action. The nature of the incident, action, or program administration that led to the complaint. The reason for the alleged discrimination (age, race, color, sex (including gender identity and sexual orientation), disability, religious creed, national origin, political belief or limited English proficiency). The names, titles (if appropriate), and addresses of persons who may have knowledge of the alleged discriminatory act. Each date on which the action occurred.
