854 Chapter 4:Working with families • Section 14: Engagement in REP Services (07/2026) to connect with others and utilize their supports. Re-engagement is a process intended to encourage participants to engage in FSSP activities. We should always ensure that each participant is treated in a manner that aligns with our Agency’s Core Values, Mission, and Vision. The re-engagement process is initiated when a concern related to the FSSP, or participation in the FSSP, has arisen. The process should be positive, participant-focused and foster their engagement in services. There should always be an emphasis on respect and understanding between all parties. The re-engagement process can be initiated by the participant, ODHS, ODHS contractor or a community partner. It is a team approach designed to holistically engage the REP-eligible participant in REP activities and possible wraparound services. The team is composed of the participant (and whomever the participant may identify as relevant to the process within their support network), and at least three ODHS department staff or designees. (e.g., Family Coach, operations manager or lead worker, engagement specialist, ODHS contractor representative, etc.) related to the FSSP. The re-engagement process is intended to: • Clarify ODHS’s understanding of the participant’s goals:
How do the planned activities support the participant’s goals?
What is not working for the participant and how we can improve it? • Identify what the participant believes could change to help them meet their goals; and • Identify what resources both the participant and ODHS can provide to support future participation. ODHS must provide the participant with a JOBS re-engagement appointment notice (7869) that must be mailed a minimum of 7 calendar days prior to the scheduled appointment. The notice should be provided in the person’s preferred language, if possible. A copy of this notice must be saved to EDMS. Sending this notice gives the participant the opportunity to participate in the re-engagement process and allows the participant time to make arrangements or request accommodations timely. A copy of the notices can be delivered during a community visit, but this does not replace the requirement for mailing the notification. Re-Engagement and TRACS entry: A PDP is not created for REP participants. An FSSP is created for each REP participant. Narration is the Family Coach’s main tool for documenting re-engagement and showing that ODHS is meeting re-engagement compliance for REP services. ODHS must ensure that narration in TRACS regarding re-engagement for REP services participation has the following narrative components: The re-engagement summary should include: • The planned re-engagement process start date and end date • The time and location of the re-engagement appointment • Interpreting services arranged for the appointment, if applicable • The status as to whether the community visit was attempted and completed • The re-engagement outcome
