626 Chapter 3:Processes and procedures • Section 2: Verification (07/2026) Verification Introduction Individuals must provide verification to ODHS|OHA when it is requested. All requests for verification must be made in writing, and the participant must be given at least 10 days to provide the requested verification. At initial application and renewal, the Request for Information (RFI) or Notice of Pending Status (DHS 0210) is used. When changes in income are reported that require verification, the RFI or Notice of Information or Verification Needed (DHS 0210a) is used. Verification may be received in a variety of ways. Verification may be: • A document that is uploaded into the electronic file, • Information received via a telephone conversation, • A document viewed during a home visit, or • Information verified through electronic sources such as DHR screens and online systems and, in some instances, can include the Federal Data Service Hub (FDSH), an interface within ONE. When verification is not uploaded in the file or available electronically, or when self-attestation is accepted, the worker must carefully narrate the information received in the case notes. If staff determine the individual doesn’t have, and can’t get, verification due to a hardship, the verification value “no verification due to hardship” can be used. This should be a rare circumstance and a case note needs to be entered to substantiate the reason it’s being used. For most situations, third party verification is the preferred source. When verification is received from a third party, the third party must have direct knowledge of the information and cannot be a member of the filing group. In certain situations, staff must have an MSC 3010 Authorization for Disclosure, Sharing and Use of Individual Information form signed by the individual or their authorized representative before requesting verification from a third party. We should not request documentation if we can verify the information electronically or if the rules allow for self-attestation. If ODHS|OHA is unable to verify information electronically or from a third party, ODHS|OHA will accept, on a case- by-case basis, self-attestation to verify all eligibility criteria except citizenship and immigration status under the following circumstances: • Documentation does not exist at the time of the eligibility determination, or • Documentation is not reasonably available at the time of the eligibility determination, such as in the case of homelessness, domestic violence or natural disaster.
