With a few exceptions, each applicant, including children, requesting medical benefits shall provide a valid SSN or, if the individual does not have one, to apply for an SSN and provide it when it is received. Yes. An individual is not required to apply for or provide an SSN if the individual: 1. Is a member of a religious sect or division of a religious sect that has continuously existed since December 31, 1950 and the individual adheres to its tenets or teachings that prohibit applying for or using an SSN, 2. Is a newborn that is assumed eligible based on the eligibility of the mother of the newborn and who is under one year of age, 3. Is a non-citizen and they: (a) Do not have an SSN and may only be issued an SSN for a valid non-work reason, or (b) Are not eligible to receive an SSN, including because they do not have a documented status. A non-applicant may provide an SSN on a voluntary basis. Oregon Administrative Rule(s) 461-120-0210 — Requirement to Provide Social Security Number (SSN) 414-175-0020 — General Eligibility Requirements 410-200-0210 — Requirement to Provide Social Security Number Address Addresses are an important part of communicating with participants. lf a mailing or physical address is out of state, it is essential to review residency in depth during an interview (when an interview is required by the program) or when speaking with the individual. Although individuals are not required to have a fixed mailing address, they must provide a mailing address as part of the application process as a location to receive notices from ODHSIOHA. This should be a safe mailing address, but cannot be a branch address. A mailing address may be General Delivery or the address of a shelter or a friend. lf an unacceptable address is provided on an application, ODHS accepts this for establishment of a filing date and collects a useable address during the interview. For medical programs, a physical address is also collected to determine which coordinated care organization(s) are available to the participant; however, only the state, county and zip code are required. Therefore, both a mailing address and at least the state, zip code and county where the applicant physically resides needs to be collected.
