1306 Chapter 8: Examples • Examples (07/2026) Payment verification Example 1: Sonoma is applying for TA-DVS. The worker can see that Sonoma has applied multiple times in the past and has always claimed that she is pregnant. The worker verifies with Sonoma that she did not give birth to any children. Payment requests will be pended for pregnancy verification from a medical professional. For the benefit of safety planning for Sonoma, let her know that before we can make any payments, we will need verification of her pregnancy from a medical professional. This gives Sonoma a chance to get verification before she needs a payment. Example 2: Doreen turned in a rental agreement as request for payment. ODHS reviews the rental agreement and check’s the county assessor website. ODHS determines the person listed as the landlord on the rental agreement does not own the home. ODHS calls the homeowner and discovers that the listed landlord is not authorized to sublease the home. Doreen’s payment request is denied because once ODHS verified all information in her request, we learned the landlord was trying to help sublease to Doreen, without owner permission. Example 3: Layla has recently fled from an unsafe situation and has been able to find new safe housing. Layla reaches out to her family coach and requests essential household items, as she had to leave behind all household items when she fled. Layla requests a couch, queen bedframe and mattress, twin bedframe and mattress, two dressers, a kitchen table and chairs. The total amount for all items is $1100. Layla shares that she looked into community resources but was unable to find essential items available. The family coach determines that the total amount requested for all items is reasonable and approves the payment. Example 4: Sammy reaches out to his family coach, as he and his children were only able to flee with the clothes they had on when they left their unsafe situation. Sammy requests clothing for himself and three children, for essential clothing items, pants, shirts, underwear, socks and jackets. Sammy shares he was able to find shoes at a community resource. The total amount in Sammy’s cart online at Walmart for all four people is $300. The family coach determines that these items are essential, that Sammy utilized community resources and the amount is a lower cost option, and approves the payment. Issuing payments Example 1: Honor has come to the office and requested a payment for a locksmith to help change out her locks quickly. The worker determines that the payment request will help meet her safety. The need to have this done today is due to the emergent situation and the worker verified that the costs to change the locks is reasonable. After verifying with the locksmith, the worker will add the payment request in ONE for a check out of office paid to the survivor. The worker explains that once the check is issued and given to the survivor, the check is now considered the property of the survivor. Allowable payments Shelter and relocation needs Example 1: Carol was approved for TA-DVS. She intends to remain in her home because her abuser has been arrested for theft and domestic violence. Her abuser broke into her house, physically assaulted her and took $500.00 she had set aside to pay her utilities. She has a utility shut-off notice for water and needs $200.00 to make the payment. Since the reason the individual cannot pay the utilities is related to the abuser’s actions, we can
