1304 Chapter 8: Examples • Examples (07/2026) for two weeks and then he came back and the abuse heightened. By asking about if this happened before and understanding the pattern, we can see why Brittany is concerned even though Francis has not talked to her in a week. Safety plan Example 1: Jordan applies for TA-DVS and is approved against her husband, Leo. Jordan reported that she has a job and will be getting paid next week. Jordan said she wants to move to a location that Leo does not know and will need the grant to help with move-in costs. Jordan also said she wants to file for divorce. She is referred to the Co-Located DV advocate to go with her to get the divorce paperwork and for further safety planning. Jordan’s plan on the ODHS 1543 is to look for a safe location, move into a new home that Leo does not know the location of, and to work with the co-located DV advocate. Jordan will use her paycheck to pay some of the move-in costs. Example 2: Rachel is in the office applying for TA-DVS, and she is approved. She creates a TA-DVS safety plan (ODHS 1543) to move to another city in Oregon, to be closer to her mom. Her immediate safety plan is to contact her mother and start talking with her, work with a DV advocate to create a plan if the abuser shows up at her home before she can move, and talk to the children’s school about transferring. Her long-term safety plan is to find a location near her mom and move. Rachel will start working once she moves and plans to use the TA-DVS grant for move in costs and a U-Haul. Name of abuser Example 1: Jona is in the office applying for TA-DVS and she is asked the name of the abuser. Jona is reluctant to share their name. Jona’s worker explains how and why ODHS|OHA documents the abuser’s name, then explains that we do not notify any reporting agencies. The works also shares the reason we ask is so ODHS can ensure payments made support her safety and independence from the abuser, for example if the abuser contacts Child Welfare and makes a false report, we would have in our case notes their name and the potential tactics they use. The worker asks Jona if they still have a safety concern with sharing the abusers name, expressing it is not required if unsafe to do so. Chapter 5, section 7: Waiving eligibility requirements What requirements can be waived Example 1: Aliah is in the office applying for TA-DVS for herself and her daughter. She came to Oregon on a student visa 16 years ago. She has remained in the US beyond her visa certification period. Her daughter was born in Oregon and is a US citizen. Aliah does not meet citizenship criteria however due to having a domestic violence related safety concern, the worker applies the ‘Citizenship or Non-Citizen status is waived’ eligibility waver. (Hint: All payment requests when citizenship is waived must be issued under payment type ‘TA-DVS Citizenship Waived’ to ensure correct funding is used.) Example 2: Lisa is in the office applying for TA-DVS for herself. She reports she is pregnant with her first child
