However, ODHS inadvertently gave Bob a card with the wrong case number on it and there were TANF and SNAP benefits on it belonging to another person, Sammie. Thinking they were his benefits, Bob uses the cash and SNAP benefits, depleting the accounts. Sammie discovered her TANF and SNAP benefits are gone from her EBT account and contacted ODHS. Once ODHS realized the mistake, the worker deactivated Bob’s EBT card that was issued in error, notified Bob of the mistake, issued a replacement card and offered Bob the options of getting it by mail or picking it up. Bob chooses to pick up a new EBT card tied to his ONE/EBT case at the local office. A replacement EBT card was also issued to Sammie and the worker replaced the amount of lost food benefits selecting ‘Agency Error’ on the Request Reason field on the Manual Issuance Request - SNAP screen and the lost TANF benefits using the Manual Issuance Request – CASH screen. Example 2: Tobey reported on May 15 that his brother, William, has left the household. ODHS|OHA removed William from the case effective May 31 but failed to deactivate William’s EBT card. In June, William continued using the EBT card and spent down Tobey’s June benefits. Because ODHS|OHA failed to deactivate William’s EBT card accessing Tobey’s benefits, ODHS must now deactivate William’s EBT card and restore the lost benefits to Tobey for June.
