The risk assessment will evaluate providers’ performance in several key areas during the fiscal year, ensuring comprehensive monitoring and adherence to program standards. The assessment content and scoring rubric are in the process of being finalized. Risk level will be determined using the following ranges: High Risk = 0- 33, Medium Risk = 34-66, Low Risk = 67-100. If the risk assessment score is 50 or above, a full management evaluation will be conducted within 60 days of the failed risk assessment. We will offer technical assistance to providers for all non-compliance. If the risk assessment score is above 50, the MTAJ-NC Consultants will continue to monitor the provider via risk assessment annually.
