How do I complete the survey online? To begin the process of completing the survey Go To: https://survey.ncsu.edu/childcare to login. Here is your login and password that you will need to gain access to the survey: Login: 20951765 Password: rg48

What if I cannot complete the survey online? If you would like for NCSU CUACS staff to call you to complete the survey by telephone, check the box indicating your desire for telephone contact on the enclosed postage-paid postcard and return the postcard via US Postal Service mail.

If you prefer to print your own paper copy of the survey, Go To: https://survey.ncsu.edu/childcare/final.pdf.

If you would like to complete your survey on paper and return that completed survey, you can return your completed survey by fax (919-515-3642) or emailed scan (rodney_oneal@ncsu.edu) or by mailing your completed paper survey via US Postal Mail to: Center for Urban Affairs and Community Services, Campus BOX 7401, Raleigh, NC 27695-7401.

** If either one of the following applies to your facility, you do not have to complete the survey: **

1. Facility was permanently closed on or before August 31, 2023;

-OR-

2. Facility did not offer: ▪ Facility DID NOT offer FULL-TIME (32 hours or more a week) child care services for children ages 0-5 in September 2023, AND ▪ Facility DID NOT offer FULL-TIME summer care for school age children in July 2023.

If either option 1 or BOTH portions of option 2 above applies to your facility, simply check the box indicating that on the enclosed postage-paid postcard and return the postcard via US Postal Service mail.

As always, the results of this survey will be kept completely confidential. Information that is specific to your center or home will never be used in reports or discussed in meetings. To make sure your information is kept private, North Carolina State University will use tracking numbers that are different from your child care program identification numbers to label your response. DCDEE will not have access to or use survey information that is specific to any single child care home or center.
