Child Care Assistance Program Policy Manual Issued 07/2021 Accuracy Review Reporting 16.44 DHS identifies specific policies for review in each quarterly review period. Each policy is reviewed for a minimum of two reporting quarters and continues until an acceptable level of error is achieved. DHS randomly selects cases to be reviewed in each quarterly review period. Agencies must report accuracy review information for selected cases to DHS. Notification to CCAP agencies DHS will inform CCAP agencies when a case is selected for the reporting quarter. Information about cases selected and policy review required is sent to agencies approximately once a quarter. The cases selected are posted on SIR. The number of cases to be reviewed by each agency is based on the caseload size of the agency. Agency reporting Accuracy reviews are reported to DHS using the Child Care Assistance Program Accuracy Review Form DHS-7356 (PDF). A new form will be created for each policy reviewed. The forms are posted on eDocs. CCAP agencies must complete a form for each case selected in each reporting quarter. The person completing the form must not be the same person that determined eligibility or processed the most current redetermination on the case. Submission timeline Accuracy review forms must be submitted for all cases selected to DHS within thirty days of DHS notification. Internal review for best practice Agencies are encouraged to conduct their own reviews of policies used when establishing eligibility, authorizing care, registering the provider or making payments to improve case accuracy and address common worker errors.
