Note: Before adding a provider assignment to a newly added child, obtain a new MDHHS-4025. Example 1: In October 2025, a newborn is added to an existing CDC program with three children already approved. The newborn is added to the case four months after the last Redetermination date (June 2025). CDC will be extended for all existing children on the case. CDC is now approved for all the children through October 2026. The 1010 for the case will now go out in September 2026. Example 2: CDC was originally approved in November 2024. In June 2025, a newborn is added to an existing CDC program with two other children, ages 10 and 12. The 12-year-old does not require constant care. The 12-year-old’s birthday is in October 2025.
