* Living in the home must be verified and documented at application, redetermination, the midpoint of a 24-month certification, and when adding a person to the certification.

B-541-FITAP – PO HOW TO DOCUMENT LIVING IN THE HOME

Documentation of “Home” must include the following:

● the source of the verification used,

● the complete name of the person providing the verification

● the date the information was provided/received,

● the address of the home,

● the names of children and adults residing in the home,

● the relationship of the source to the client,

● the contact number for the source, and

● if applicable, the reason for the temporary absence of the qualified relative or child. **
