Living in the home must be verified at application, redetermination, the midpoint of a 24-month certification, and when adding a person to the certification.

Verification must be current and must contain sufficient information to establish that the child and qualified relative live together.

Use the following sources for verification:

● School records, school bus drivers

● Hospital, clinic, Health Unit, or private physician's records

● Juvenile court records

● Other agencies' records

● Church records

● Landlord's statement, verbal or written

Note: Landlord is someone who leases or rents property to another person.

● Child care provider

● Home visit

● * OFS 80 Collateral Statement

Note: If an OFS 80 is used, the collateral must be verbally contacted and the statement verified. **

● Verbal statement from a knowledgeable collateral contact when other verification is not available.
