During the exit conferences, as areas of deficiency are discussed, all parties, including the Management Review Officer (MRO), Program Assistance Program Specialist (PAPS), Service Region Administrator Associate (SRAA), Family Support Supervisor (FSSV) or designee, and local office staff, contribute to the plan development. Regional staff may also request a revision to the CAP.
CAP plans are completed as follows:
A. Name of Service Region.
B. Date: This is the date the CAP is completed by the Department for Community Based Services (DCBS).
C. Indicate the County, Unit, or Branch for which the plan is being made.
D. Description of Deficiency: ME indicates the deficiency area addressed. DCBS identifies the root cause and error element.

G. Tasks/Steps, timeframes, and staff responsible for each step: DCBS lists the tasks/steps, timeframes, and staff responsible needed to implement the activity in F.
H. Description of plan to monitor activity: DCBS must explain how the tasks/steps will be monitored to ensure they have been implemented.
