11121.1 Agency Error

Instances of agency error which may result in a claim include, but are not limited to, the following:

Prompt action was not taken on a change reported by the household.

Household income or allowable deductions were incorrectly computed.

Coverage continued after the review period expired without benefit of a required redetermination of eligibility.

Policy was misapplied.

The loss of SSI recipient status for an SI medical assistance recipient was not timely processed, including failure to timely request or process a new application or information.
