Long Term Care
When the BLTC nurse suspects abuse or neglect, the nurse notifies the BLTC Regional Supervisor who, in turn, notifies the Metro or Downstate Area Manager, as appropriate.
When the long term care worker suspects resident abuse or neglect, they are to notify the BLTC Regional Supervisor. Use the Problem Resolution Request form.
In emergency situations telephone the BLTC Regional Supervisor. The phone number can be obtained by calling BLTC at 217-782-0545. The Regional Supervisor notifies the BLTC nurse. The nurse follows-up on the report made to DPH and reports back to the local office.
Reports of suspected abuse or neglect may also be made to DPH in writing or in person. The person making the report is not required to give their name.
