Call the Department of Public Health (DPH) Long Term Care/Nursing Home Hotline (1-800-252-4343) to report suspected abuse or neglect of LTC residents. The hotline operates 24 hours a day and is toll-free.

Call the Department of Healthcare and Family Services (HFS) - Bureau of Long Term Care (BLTC) (217-782-0545) to report suspected abuse or neglect of SLF residents. Follow-up in writing.

The report should include:

name of resident;

name and address of the facility at which the resident resides;

resident's age, if known;

date and time of the suspected abuse or neglect, if known;

nature of resident's condition, including any sign of previous injuries or disabilities;

any other information that might be helpful in establishing the cause of the abuse or neglect; and

identity of the person believed to have caused the abuse or neglect.
