Proof of Applicant Identity Acceptable documents include one of the following:  Government-issued ID;  Documentation, such as public benefits award letters, medical records, school records, or other documents that reasonably verify identity issued within the last 12 months; or  When documentation cannot be provided, the applicant can sign a written statement to self-verify their identity.
