8 improvement and accuracy. Additionally, HFC has deployed targeted follow-up surveys for adults age 60+ and households that are prenatal up to having children of 3 years of age. These surveys focus on tech, mobility, and health-specific obstacles. Analysis of these surveys is leading to the goal of focusing closer on these obstacles for all SNAP eligible Coloradans. County Coordination HFC navigators and specialists will accurately gather all important case details, including the case number, county of residence and a clear description of the client issue before determining if an escalation is warranted. Should an escalation be warranted, HFC will escalate the issue to the assigned county using Salesforce and PEAKPro data. All escalated cases will be monitored closely with follow-up within 7 business days. All escalated client cases will remain open in Salesforce until a response is received from the county or until a verifiable update is reflected in PEAKPro. Other FNS Referrals HFC will ensure clients have access to information on all available support services provided by FNS. Navigators and specialists will assess the individual's needs during interactions and provide information or make adequate referrals to relevant federal programs. Referrals will be completed in a timely manner, with follow-up as needed by the navigator to confirm connection to services. Any shared information or referral made will be clearly documented in the client’s case in Salesforce. Comprehensive Training HFC will deliver comprehensive training to over 200 navigators at 46 statewide community partners in Colorado on SNAP Outreach, eligibility criteria, application assistance, and PEAK Pro through an online learning platform, as well as effective methods for evaluating and reporting program outcomes, from October 2025 through September 2026. Within the learning platform, there are quizzes and knowledge checks that simultaneously provide HFC with information on the progress their subrecipients are making and gaps in their learning. Their SNAP PEAS navigators include individuals from various linguistic, learning, and cultural backgrounds, and they created the platform with this in mind. The learning modules offer various ways to absorb learning content, including videos, audio, visuals, and written text. Additionally, the entire platform is available in Spanish to make learning more accessible for the numerous navigators whose preferred language is Spanish. Their learning modules are designed to cater to most learning styles and are presented in bite-sized pieces, allowing individuals to complete the training at their own pace. Most participants complete the training within a week. SNAP PEAS In FFY 2026, HFC will collaborate with 47 subrecipients (SNAP PEAS) across Colorado to expand SNAP outreach, increase program impact, and improve access to nutrition assistance for eligible populations. Each partnership is established through an individual contract between HFC and the subrecipient, with funding provided by HFC. Grants are awarded based on each subrecipient’s capacity, the populations and communities they serve, and their geographic location. These grants are tied to annual application goals, which are determined by the amount of funding received. The contracts outline all key details, including the budget, allocated funds, application targets, and compliance requirements. The contract also provides access to PEAK Pro Licenses, which is necessary for assisting clients with SNAP applications. CDHS Collaboration
