ASSESSMENT Does the State require or provide an assessment? Yes No Who conducts the assessment? Select all that apply. State Agency E&T Provider Self-Assessment Intermediary Local Office Other When are participants assessed? The comprehensive employability assessment is a critical step in the EF case management process and is completed by the EF PO during orientation or the initial appointment. Assessments are completed in person and/or virtually between the client and the EF PO by completing either the State's EF Comprehensive Employability Assessment and the Transferable Skills Checklist or an EF PO’s assessment tool that is approved by the State EF program team. Assessments help determine the appropriate E&T component (commonly referred to as a program service) the client can engage in, as well as assess for barriers to employment. Areas that are assessed include employment history, education, and the client’s short- and long-term goals. Assessments must be conducted at least annually; however, EF POs assess clients at least monthly for barriers to employment and the need for supportive payments or updates to the client's individual employment plan. Describe the assessment. List the tools used in the assessment. Both the comprehensive assessment and transferable skills checklist are used to assess different areas such as communication skills, planning skills, interpersonal skills, critical thinking skills, academic skills, and self-management skills. Results of an assessment and the supportive payments that are provided (from the EF program or a partner employment and training program) to meet a client’s needs are documented in CBMS. The client is provided with the results of their assessment as well. Does the assessment result in the completion of an individual employment plan? Yes No 27
