o Complete a DFA-36B. • Place a block on out-of-state transactions on their EBT card. o To block Out-of-State Transactions: Go to ebtEDGE.com or use the FIS ebtEDGE mobile app to block out-of-state transactions. This can be unblocked at any time when needed. • Once the DFA-36B is completed and signed, the form must be sent to the Division for Family Support (DFS) unit with a printout of the questionable EBT transaction. Once DFS investigates and confirms the theft was from card skimming/ cloning/phishing, they will advise the worker of the amount that can be replaced via an email. The DFA-36B is retained in the case record along with the email from DFS containing the approval/denial. • The Case Manager will replace the stolen WV WORKS benefit amount only on the EBT card by using the auxiliary reason, replace stolen benefits due to skimming/cloning/phishing. • If the claim is found by DFS to be invalid, the worker will complete a system generated notice (NCSB) by checking the appropriate denial reason and mailing it to the client. The worker will retain the NCSB in the case record. • Replacements will be limited to the lesser amount of the benefits stolen from the household or the amount equal to two months of the households monthly allotment. • The household may only receive two instances of replacement benefits in each Federal Fiscal Year (FFY), even if an occurrence of theft crosses two FFY’s. 12.3.11 WHEN THE BA-67 IS REPLACED The BA-67 is replaced when one of the following conditions is met: • The BA-67 is not received by the client and has not been returned to the DOHS. • The BA-67 is received by the client, but is stolen, lost, or accidentally destroyed before being redeemed. When a replacement BA-67 is issued and the client subsequently finds and redeems the original BA-67, he must reimburse the amount of the replacement BA-67. Until the amount is repaid in full, the client is ineligible for any future replacements.
