Also, the second sequence of numbers refers to the State of West Virginia fiscal year- 2020. As of July 1st, of each year, the fiscal year changes, so the fund code changes to reflect the fiscal year. As of July 1, 2020, please change to 2021 and change it each following year while we continue to do this by hand. Once completed, the form and the appropriate returned check or payments shall be submitted to the closest DoHS office. 14.5.7. BCF-PSRC Authorization and Release for Protective Services Record Check This form will be given to the potential child care provider for completion. (One copy for each member of the household over the age of 18.) The form is to be returned to the CCR&R office for processing.
