10.5.1 W-2 Payment Designation

CSJ Community Service Job , W-2 T W-2 Transition , or CMF+ Case Management Follow-up Plus payments must be made payable as appropriate to the:

Participant;

Spouse of the participant (the spouse must be living in the home unless designated as protective payee or appointed by a court to be legal representative); or

Guardian or conservator of the W-2 participant.
