2500.9.2 Replacing EBT Cards

EBT cards are replaced through the EBT unit.

Follow the steps below when households report their EBT card was lost, stolen or damaged: Verify the household’s mailing address is correct in EBT Edge . If the address is correct, give the household the EBT customer service number (1-800-914-8605) to report their card was lost, stolen or damaged. If the mailing address is incorrect, send an email to the EBT unit with the household’s correct mailing address. Write "LAST NAME Address Change & Card Request" in the subject line of the email. Request the EBT Unit: update the mailing address, and replace the EBT card for the household. CATN your actions.
