I. Policy:

Only primary contract infant formulas, exempt infant formulas and medical foods approved for use in the WIC Program may be prescribed in a food package.

II. Procedure(s):

A. Refer to the website WIC Infant Formula for the complete listing of approved primary contract infant formulas, exempt infant formulas and medical foods.

B. To prescribe a food package containing a contract infant formula refer to FDS 03.1.

C. To prescribe a food package containing an exempt infant formula or a medical food refer to FDS 03.2.
