I. Policy:

Local agencies shall provide participants, parents / legal guardians, and caretakers with information on Medicaid and other health-related or public assistance programs, and when appropriate, shall refer participants to such programs.

II. Procedure(s):

A. Provide written information on the Medicaid Program, for Medicaid Eligibility Guidelines, and other social services programs available within the local community to every participant, parent / legal guardian or caretaker at each certification and subsequent appointment.

1) The written information shall include the name, address, phone number, and specific services which each program or service provides.

B. Document all referrals in Crossroads at each certification and subsequent appointment.
