Financial and Supplemental Nutrition Assistance Program (SNAP)
Customers who know their Social Security Number (SSN) can tell the eligibility specialist their number.
If the SSN is questionable, or the customer does not know the number, one of the following will be required:
An official SSN card; or
Other official documents from the Social Security Administration (SSA) which identify the person’s correct SSN. These include, but are not limited to, award letters, Third Party Queries (TPQYs), and benefit checks; or
An SSA receipt (form 5028 or 2880) on which SSA lists our ID number for the person.
When a customer cannot submit a correct SSN as above, the customer can submit one of the following types of documents to show they have applied for an SSN:
A receipt from Social Security (5028, 2880, or computer letter verifying application for social security number).
A birth certificate or a Form SSA-2853 showing that the parent has requested a Social Security card.
If the customer cannot provide the correct SSN verbally, with an SSN card or other SSA document, they must apply for an SSN card.
