Parents
Select an approved provider. Customers must report their choice of provider to the Department of Workforce Services (DWS) along with the start date of care if known. If no start date is reported or there is a discrepancy in start dates, the Department may use the start date verified with their chosen provider. Establish with the provider the individual need and cost of care per child. Report changes in providers timely. Complete and submit any requested Child Care verifications.
Any reported changes in the child care provider must be reported/or verified by the customer before it can take effect on the case.
The parent is responsible for paying the co-payment amount and all costs of care charged by the provider. If the Child Care subsidy is less than the amount charged by the provider, the parent is responsible for paying the child care provider for the difference.
The customer is responsible for monitoring the child care provider. DWS will not monitor the provider.
