603 CHIP Member ID Cards
Effective Date: July 1, 2018
No Previous Policy
CHIP member ID cards are issued by the health and dental plans the member chooses to enroll in. The member must select their plans before the card will be issued. The card is a wallet size, plastic card and will include the individual’s name, PID and date of birth. Providers will use the member information on the medical card to verify eligibility and benefit information.
Once the member selects a health and dental plan, they will receive their benefit cards from those plans. Members will need to retain the card for future months.
o Each child enrolled in CHIP will receive their own cards.
If the member has an urgent medical need prior to receiving their card, they can contact the health or dental plan they enrolled in for assistance.
If a new child is determined eligible and added to the open CHIP case, they will receive their benefit cards once they select a health and dental plan to enroll the child in.
Members must contact the plans if they have not received their cards within 6 weeks or they have lost or misplaced their member ID cards ( 601 ).
