213 If a Prisoner match occurs, • Send CHIP Notice C315, Prisoner Match - Information Request, notifying the household that a match was received. The notice also includes instructions to the household to have the correctional facility complete the bottom half of the form or submit release documentation if the person is no longer in the facility.
• Allow 10 days for the household to submit the completed form or contact the worker. The worker must provide assistance in obtaining the verification, if requested. • If verification is not received by the due date, remove the person from the SNAP budget (or close the case for 1-person household or if all adults are incarcerated) following Advance Notice (Notice of Adverse Action) requirements. • If the ES worker decides to make contact with the correctional facility to request the release information prior to being asked by the household to assist with obtaining the verification, and is successful in obtaining the verification, CHIP Notice C314, Prisoner Match-Notice of Match Results, must be sent to the household. This notice informs the household that information has been received from the Social Security Administration indicating that a household member is or was previously incarcerated but that no further action is required of the household because the Agency has already obtained the necessary verification.
The Agency has an established system to verify and ensure that SNAP benefits are not issued to individuals who are deceased. The system provides for comparing identifiable information about all household member against information from the database on deceased individuals. The agency must make the comparison of matched data at the time of application and no less frequently than once a year.
If a Death Match occurs, • Send CHIP Notice C321, Loss of Household Member, to the household. • Allow 10 days for the household to respond.
After the 10-day time frame has expired, take action as follows: • All members deceased: Close the case. NOTE: A closure notice is not required. If the worker chooses to send a notice, send CHIP Notice C320, Closure - Death of Client. • Multiple household members with at least one adult remaining in the home and one or more members are deceased: Remove the person(s) from the case and send a notice informing the household of the change in benefits with the reason identified. (i.e. “because ______ has been removed from the SNAP case”).
